NOTE:
This application is only for candidates who are reactivating their license by examination for Oklahoma LTC/HHA or Medication Aide/Insulin Injection Aide or for those who have not tested previously with another company.
- You are eligible to challenge if:
- You have not tested previously with another company.
- You have been approved to reactivate your license by examination by the Oklahoma State Department of Health (OSDH).
- You will need to upload your OSDH approval to test letter.
- You are only allowed one attempt to pass the exams (knowledge and skills). If you do not pass, you will need to take a class through an Oklahoma State Department of Health-approved training program.
INSTRUCTIONS:
- Complete and Submit this Oklahoma LTC/HHA or Medication Aide/Insulin Injection Aide Examination Application.
- Upload one of the following required documents:
- Training Verification Form if you have not tested previously with a different company
- OSDH Approval to Test Letter to challenge the exam to reactivate your license by examination
- Once approved, you will be sent an email with your Username and Temporary Password to sign in to your TMU© account.
- Please refer to the Oklahoma LTC/HHA Candidate Handbook or Oklahoma Medication Aide/Insulin Injection Candidate Handbook for instructions on Completing your Account and other important testing policies and updates.
Once you have completed all the fields and uploaded the required documents within this application, select “Send Application" to submit your application.
